Workcover Proof of Identity Requirements

WorkCover NSW introduced changes to its Licensing scheme in 2006.  Workcover now issues photo licenses under the National Occupational Health and Safety Scheme.  You must be at least 18 years of age to sit a Workcover assessment.

PROOF OF IDENTITY

Before you can undertake an assessment under the WorkCover scheme you must provide documents of identity that add up to 100 points. The table below list identity documents and their point value.

  • The documents must be original documents. Certified copies will not be accepted.
  • Within these documents you must show your photo, date of birth, signature & current address.

LOG-BOOK REQUIREMENTS:  WorkCover changed the log book requirements on September 1 for all High Risk licenses including Forklifts.  Operators using log books must now be enrolled and supervised by a Registered Training Organisation like HPOTS.  If you have operators currently on log books or if you are operating on a log book please give us a call so that we can assist you to comply with the new requirements.

What if you can't provide these documents?

If you are not certain that you have sufficient documents for the 100point ID check contact us.  You will not be allowed to undergo the assessment unless you provide these documents.
